Percutaneous Aspiration, Lavage and Antibiotic Instillation

چکیده
Further to my recent publication in the Journal on the percutaneous management of acute calculous cholecystitis1, I would like to communicate the results of a prospective randomized study conducted at the Medical City in Baghdad, Iraq to examine the role of percutaneous aspiration of the gallbladder followed by lavage and antibiotic instillation into its lumen (500 mg ampicillin with 40 mg gentamicine, PALA) when conservative management of acute cholecystitis appears to be failing. Patients judged suitable for the study were those presenting with clinical signs and symptoms (including a positive Murphy's sign) of acute cholecystitis coupled with pyrexia (more than 38 C), tachycardia, leucocytosis, normal serum amylase levels and liver function tests, and ultrasonic evidence of a distended gallbladder containing one or more stones. These patients were treated conservatively. They were completely fasted, confined to bed, given 50 mg pethidine intramuscularly, hydrated intravenously, allowed nil orally for at least 24 hours, and intravenously treated with 1.2 grams of Augmentin (amoxycillin 1 g and clavulanic acid 200 mg, Beecham Research Labs, Brentford, U.K.) every 8 hours. When this treatment proved unsuccessful (failure of fever or tachycardia to show unequivocal evidence of remission or of tenderness and rigidity to subside within 36 hours of beginning nonoperative treatment), patients were fasted, their antibiotic stopped, and after giving written informed consent, they were randomized by drawing sealed envel

Percutaneous gallbladder aspiration for acute cholecystitis.
Early cholecystectomy for patients with acute cholecystitis may not be possible in some clinical settings. Percutaneous gallbladder aspiration (PGBA) offers an alternative approach, but the benefits and risks of this procedure are unclear. We synthesized data on the outcomes of PGBA in acute cholecystitis patients using data sources from online databases, including MEDLINE and EMBASE, and bibli...
متن کاملPercutaneous Aspiration, Lavage and Antibiotic Instillation. New Approach in the Management of Acute Calculous Cholecystitis
Distension of the gallbladder and bacterial infection can perpetuate an attack of acute calculous cholecystitis and produce its local and systemic complications. This prospective randomized trial was conducted on patients with their first episode of acute calculous cholecystitis which was associated with pyrexia and tachycardia to examine whether ultrasound guided percutaneous aspiration and la...
متن کاملPercutaneous aspiration of hepatic cysts does not provide definitive therapy.
Follow-up of 13 patients who underwent radiologically guided percutaneous needle aspiration of simple liver cysts revealed cyst recurrence in all patients within 2 years. While still effective as a primary diagnostic maneuver, percutaneous aspiration seems to lack permanent therapeutic benefit. However, it may be used as a therapeutic trial to confirm the origin of the patient's symptoms. The p...
متن کاملChanges in the Bacterial Microbiota in Gut, Blood, and Lungs following Acute LPS Instillation into Mice Lungs
INTRODUCTION Previous reports have shown that the gastrointestinal (GI) bacterial microbiota can have profound effects on the lungs, which has been described as the "gut-lung axis". However, whether a "lung-gut" axis exists wherein acute lung inflammation perturbs the gut and blood microbiota is unknown. METHODS Adult C57/Bl6 mice were exposed to one dose of LPS or PBS instillation (n=3 for e...
متن کاملme Role of Tumor Necrosis Factor - a in the Pathogenesis of Aspiration Pneumonitis in Rats
Background Aspiration pneumonitis is characterized by proteinaceous pulmonary edema and acute infiltration of neutrophils into the alveolar space. This study examined the role of the proinflammatory cytokhe, tumor necrosis factor-a (TNFa), on the pathogenesis of the injury produced by the different components that may be present in the aspirate, acid, or gastric particles. Methods: Rats were in...
